Tabs are Automatically Combining when they Should be Separate with Online Ordering from QR Code

We are experiencing an issue with online ordering from the table. It works fine most of the time but has of late started to randomly combine tabs at a table on occasion without anyone doing anything to combine them. This has resulted in customers having a tab with many items they did not order. Is anyone else experiencing this? We have checked our settings and have no new settings.
