Item Approve Dialogue and Unit Cost / Size Cost

I have a case of frozen battered mushrooms. (see attached image links below).  I need to set up the item via the approve dialogue so my recipes can calculate the cost per serving (12 to 15 fried mushrooms)...

Any pointers would be appreciated...
