The offer page seems to be very restrictive. I see create 20% off entire check, and there is a time duration. But can't specify a minimum spend amount?

You can still assign a promo code and show it online if you want, just click the display on menus link under the promo code.

The "Offer Page" is intended for people who want to create a simple quick discount without worrying about all the configurations. For more customizable options on discounts, go to "discounts and promo codes" page.
And to answer your questions:
1. Yes, it can be configured to have an %off discount applied when a set minimum is met.
2. Yes (sort of). You can create a "single-use" discount code for your discount. A single-use discount code can be applied only once per loyalty account.
3. Unfortunately, discounts cannot target chosen audience group at the moment. I personally really hope a huge implementation on loyalty/marketing/discount is the next item on Toast's roadmap. (c'mon Toast. Wink wink)
4. Yes, it can be set up like that under "Service Charges and Fees" page. https://www.toasttab.com/restaurants/admin/menus/servicecharges

1. Go to https://www.toasttab.com/restaurants/admin/#/discounts
2. Click "edit" on the discount you want to modify
3. Scroll all the way down to "advance settings" and then click "show" to reveal the options.

This is slightly different from Offers. Offers are listed on the website front page and also in the app and drive more sales, but the discount codes work differently and are not advertised online.
